New advances in the management of patients with cirrhosis

Apr. 25, 2013 ? New data from clinical studies presented for the first time at the International Liver Congress? 2013 provide new rationale for an old and established treatment option for portal hypertension. Additionally, spleen stiffness predicts the occurrence of clinical complications, which is of paramount importance in clinical practice.

In patients with cirrhosis, increasing blood pressure in the abdominal circulatory system (known as portal hypertension) leads to potentially lethal complications which might be prevented with simple medical treatment. Patients with cirrhosis and portal hypertension have increased gastrointestinal permeability which allows the movement of bacteria or bacterial components through the lining of the gut into the blood stream in a process known as bacterial translocation. Bacterial components such as lipopolysaccharide can be involved in the genesis of complications of cirrhosis.

The first study evaluated the effects of a non-selective beta-blocker (NSBB) on gastrointestinal permeability and bacterial translocation in patients with cirrhosis with high levels of portal hypertension.1 Patients with severe portal hypertension (HVPG* ?20mmHg) had increased markers of gastrointestinal permeability and bacterial translocation compared to patients with lower levels of portal hypertension (HVPG<20mmHg). Treatment with NSBB significantly reduced HVPG, improved gastrointestinal permeability and decreased bacterial translocation (LPS-binding protein (LBP) -16% p=0.018; IL-6 -41% p< 0.0001) levels.

Patients who were found to have the highest levels of gastrointestinal permeability were also found to be at most risk of bleeding from esophageal varices; a complication of cirrhosis which carries a high risk of mortality.

These findings provide a new rationale for the use of non-selective beta-blockers in patients with cirrhosis. EASL's Treasurer Prof. Mauro Bernardi commented on the data: "The movement of bacteria from the gut and into the bloodstream is extremely serious and potentially fatal in patients with cirrhosis often leading to complications or death. Beta-blockers have been successfully used in a number of conditions and as a standard treatment to control blood pressure in other disease areas. In cirrhosis, they have been used for decades for primary and secondary prophylaxis of bleeding from esophageal varices. The results of this study show that besides improving portal hypertension, as it was thought up to now, their beneficial effects are also due to their ability to reduce bacterial translocation which may widen the indications for the use of these drugs in this setting."

In the diagnostic landscape, promising data to support the validity of non-invasive techniques were also presented at the congress. HVPG, an invasive measurement technique currently considered as the best predictor to identify progression to severe scarring of the liver and disrupted essential body functions (clinical decompensation), was compared to techniques such as the evaluation of spleen stiffness (SS) combined with the MELD** score.2 ?

The study showed that in compensated (early) patients with cirrhosis both the SS (p<0.0001) and the MELD (p=0.016) score provided an accurate prediction of clinical decompensation, and their combination in a new score had a predicting power even superior to that of HVPG.

Prof. Mauro Bernardi added, "HVPG is an invasive technique, which can often be discomforting for patients, is only performed in specialised centres and needs experienced operators to be fully reliable. While further studies will be required, if non-invasive techniques continue to present accurate predictions, they would be welcomed in the overall management of compensated patients with cirrhosis."

It's not easy material but it's truly fascinating, and expertly done.

An extremely perceptive and intriguing examination of the effect that media hype and spin have on the political process.

Larrain deftly mixes social satire and historical drama.

All historical and little drama.

Larrain does a fine job of making No look and sound authentic to its time period, although the VHS-quality photography, all washed-out with colors bleeding together as camcorders did in the '80s, is an occasional irritant.

Silliness is on the side of the angels in a brilliant and highly entertaining film that's part political thriller, part media satire.

It's clear that the language of advertising has become universal, and that political commodities can be sold like soap. But toppling a dictatorship? Now there's a story.

A reflection of a moment in time, made in the image of that moment.

Bernal deftly explores the layers of the character's complexity, including his political apathy.

"No" is filmmaking of the first order.

Old technology plus the packaging of a revolution add up to a Yes

Freshens up a decades-old story with vibrant humor and a good sense of storytelling.

No continually impresses for its slyness and savvy -- rarely has such an eyesore been so worth watching.

Larrain fashions an unlikely crowd-pleaser from a historical episode that has its share of tragedy as well as triumph.

American Airlines to expand flights from Los Angeles hub

(Reuters) - AMR Corp's American Airlines on Wednesday said it plans to expand service from Los Angeles, one of its five major U.S. hubs that is an important business and leisure market, by adding flights to nine cities this year.

American, which plans to merge with US Airways Group Inc and form the world's biggest carrier, said the new destinations from Los Angeles will bring additional flight options to nationwide passengers and enable it to connect more travelers to international markets.

New cities to be served from Los Angeles starting this summer include Pittsburgh; Columbus, Ohio; Indianapolis; and Hartford, Connecticut. Through agreements with feeder carriers, American will also add service to Bentonville, Arkansas, and the Oregon cities of Eugene and Redmond from Los Angeles.

American said service from Los Angeles to Sao Paulo is due to begin in November pending U.S. government approval of its application to add more flights to Brazil from the United States.

In April, American began flying to Raleigh/Durham, North Carolina, from Los Angeles. As a result of the expansion, American will fly to and from 51 U.S. and international cities from Los Angeles.

Virasb Vahidi, American Chief Commercial Officer, said the expansion would strengthen the presence of the oneworld global airline alliance, which is looking to fill gaps in its worldwide network and catch up to rival alliances SkyTeam and Star. Oneworld is led by American and British Airways .

The expanded Los Angeles service "provides a very unique opportunity for us to be able to capture a disproportionate share of high-value customers and corporations," Vahidi said. Other oneworld member airlines such as Japan Airlines, Qantas, Cathay Pacific and BA also operate in Los Angeles.

At Los Angeles International Airport, American is second in passenger market share with 16.3 percent, behind United Continental's 17.6 percent share, according to data for January 2013 through February 2013 provided on the facility's website.

Los Angeles is one of five key U.S. hubs that American, which filed for Chapter 11 bankruptcy in November 2011, is counting on to drive growth. The others are Dallas/Fort Worth, New York, Miami and Chicago.

Microsoft Xbox Next ?Durango? May Cost $500 ? Tech Analyst - X-bit ...

A renowned technology analyst has revealed that while Microsoft?s Corp.?s next-generation video game console will be heavily based on PC technology, it will cost more than an average personal computer. The reasons for Xbox code-named Durango are not completely clear, given the fact that it will compete against mobile devices, PCs and Sony PlayStation 4.

"Durango is going to be expensive ? $500, $300 with a subscription ? that kind of thing. Originally, they were going to announce this thing on April 24. Now they are going to announce it on May 21. We know there are events occurring this year where we are going to learn more about Durango. E3 is going to occur, Build is going to occur in San Francisco in June when they are going to talk about the developer story because it is a Windows 8 device. It is going to have the same, or basically the same, developer tools and developer APIs," said Paul Thurrott, a well-known Microsoft expert, in the recent What The Tech's video podcast.

Microsoft released the Xbox 360 console in the U.S. back in 2005 at $299 and $399 price-points and it has taken the company quite some time to slash pricing of the product to mainstream $199. With models priced at $399 and $499 the software giant will hardly be able to outsell the Xbox 360 game console at least early in the lifecycle. Mr. Thurrott also revealed that according to his sources, the console would require a constant internet connection

It is also rumoured that Microsoft intends to release a low-cost Xbox 360 version code-named Stingray at $99 price-point later this year, bringing value gaming and advanced entertainment capabilities to customers in budget. While the company is clearly interested in such an offering, given that modern Xbox 360 games cost $50 and higher, the Stingray will barely get popular among casual gamers, but will rather be a substitute for those, who want to play current titles that will not work on Xbox Next.

Microsoft Xbox Next ?Durango? is expected to be architecturally similar to the PlayStation 4. It is believed that the future Xbox will be powered by AMD Fusion custom-designed system-on-chip with eight x86 low-power/low-cost Jaguar cores, AMD Radeon HD graphics with GCN architecture as well as 8GB of DDR3 system memory. The console is projected to feature hard disk drive, Blu-ray disc drive as well as robust Xbox Live online service. It is believed that Microsoft Xbox ?Durango? has lower-performance graphics sub-system as well as slower memory sub-system when compared to Sony PS4.

That is not what they mean. 'Always on' means when the console is on you need to be connected to the internet for it to function correctly.

You can turn the power off when ever you want.

But you are right that it means DRM, but I really don't have any issues with DRM as long as it doesn't massively impede on my system performance. I am more than happy to wait for a game to come down to a price I feel it is worth. The game makers have put a lot of time and money into their games and if we want good, high quality games we have to accept to part with our money. I also think second hand games should be banned too. If there was no piracy and no second hand games then possibly the game price would come down but mainly the games companies would make more money which on the whole will lead to more and better games.

The bigger problem is that Durango will be tightly integrated into Microsoft' ecosystem and will not use any exclusive technologies. Therefore, "always on" requirement and constant DRM will likely be a part of all Microsoft's products going forward.

Was Chilean poet Neruda murdered?

ISLA NEGRA, Chile (Reuters) - The body of Chilean Nobel laureate Pablo Neruda, dead nearly four decades, was exhumed on Monday after his former driver said the poet was poisoned under Augusto Pinochet's dictatorship.

Neruda, famed for his passionate love poems and staunch communist views, is presumed to have died from prostate cancer on September 23, 1973.

But Manuel Araya, who was Neruda's chauffer during the ailing writer's last few months, says agents of the dictatorship took advantage of his illness to inject poison into his stomach while he was bedridden at the Santa Maria clinic in Santiago.

"We're hoping for a positive result because Neruda was assassinated. Pinochet made an error when he ordered Neruda be killed," said Araya. Results are expected in coming months.

Neruda was a supporter of socialist President Salvador Allende, who was toppled in a military coup on September 11, 1973, nearly two weeks before the poet's death at age 69. Around 3,000 people are thought to have been killed by the brutal 17-year-long dictatorship that ensued.

Neruda was buried in his coastal home of Isla Negra beside his third wife, Matilde Urrutia.

His remains will be brought to Santiago for analysis. Some samples could be sent to laboratories abroad.

Ricardo Eliecer Neftali Reyes Basoalto, better known by his pen name Pablo Neruda, was a larger-than-life fixture in Chile's literary and political scene.

While best known for his collection "Twenty Love Poems and a Song of Despair," published in 1924, Neruda was also an important political activist during a turbulent time in Chile.

He organized a ship to bring about 2,000 Spanish refugees fleeing the civil war there to Chile in 1939, campaigned for Allende and was ambassador to France during the socialist's presidency.

The Andean country's intelligentsia frequently congregated in Isla Negra, as well as in his Santiago home "La Chascona" - so named for his then-mistress Urrutia's messy red hair - and La Sebastiana, his ship-themed home in the port town of Valparaiso.

Democratically elected Allende committed suicide in the presidential palace as it was under attack by the military, experts confirmed last year, amid accusations he had been murdered during the coup.

Chilean courts are also investigating the death of ex-President Eduardo Frei Montalva, who is presumed to have died in 1982 of an infection after a hernia operation. Some say he was poisoned by Pinochet's agents.

Engineered T cells kill tumors but spare normal tissue in an animal model

The need to distinguish between normal cells and tumor cells is a feature that has been long sought for most types of cancer drugs. Tumor antigens, unique proteins on the surface of a tumor, are potential targets for a normal immune response against cancer. Identifying which antigens a patient's tumor cells express is the cornerstone of designing cancer therapy for that individual. But some of these tumor antigens are also expressed on normal cells, inching personalized therapy back to the original problem.

T cells made to express a protein called CAR, for chimeric antigen receptor, are engineered by grafting a portion of a tumor-specific antibody onto an immune cell, allowing them to recognize antigens on the cell surface. Early first-generation CARs had one signaling domain for T-cell activation. Second-generation CARs are more commonly used and have two signaling domains within the immune cell, one for T-cell activation and another for T- cell costimulation to boost the T cell's function.

Importantly, CARs allow patients' T cells to recognize tumor antigens and kill certain tumor cells. A large number of tumor-specific, cancer-fighting CAR T cells can be generated in a specialized lab using patients' own T cells, which are then infused back into them for therapy. Despite promising clinical results, it is now recognized that some CAR-based therapies may involve toxicity against normal tissues that express low amounts of the targeted tumor-associated antigen.

To address this issue, Daniel J. Powell Jr., PhD, research assistant professor of Pathology and Laboratory Medicine, Perelman School of Medicine, University of Pennsylvania, and director of the Cellular Therapy Tissue Facility, developed an innovative dual CAR approach in which the activation signal for T cells is physically dissociated from a second costimulatory signal for immune cells. The two CARs carry different antigen specificity -- mesothelin and a-folate receptor. Mesothelin is primarily associated with mesothelioma and ovarian cancer, and a-folate receptor with ovarian cancer.

Powell likens this dual CAR approach to having two different gas pedals, one for starting the immune system and a second for revving it up. Dual CAR T cells are more selective for tumor cells since their full activity requires interaction with both antigens, which are only co-expressed on tumor cells, not normal tissue.

Dual CAR T cells showed weak cytokine production against target cells expressing only one tumor-associated antigen in lab assays, similar to first-generation CAR T cells bearing the CD3 activation domain only, but demonstrated enhanced cytokine production upon encountering natural or engineered tumor cells expressing both antigens, equivalent to second-generation CAR T cells with dual, but unseparated signaling.

In a mouse model of human ovarian cancer, T cells with the dual-signaling CARs persisted at high numbers in the blood, accumulated in tumors, and showed potent anti-cancer activity against human tumors. Dual CAR T cells were equivalent to second-generation CAR T cells in activity against tumors bearing two antigens. However, the dual-signaling CAR T cells did not react vigorously with normal tissue expressing one antigen while second- generation CAR T cells did.

"This new dual-specificity CAR approach can enhance the therapeutic efficacy of CAR T cells against cancer while minimizing reactivity against normal tissues," says Powell.

Their findings have been published in the inaugural issue of Cancer Immunology Research, the newest journal from the American Association for Cancer Research.

GENEVA (AP) ? It is one of the cosmos' most mysterious unsolved cases: dark matter. It is supposedly what holds the universe together. We can't see it, but scientists are pretty sure it's out there.

Led by a dogged, Nobel Prize-winning gumshoe who has spent 18 years on the case, scientists put a $2 billion detector aboard the International Space Station to try to track down the stuff. And after two years, the first evidence came in Wednesday: tantalizing cosmic footprints that seem to have been left by dark matter.

But the evidence isn't enough to declare the case closed. The footprints could have come from another, more conventional suspect: a pulsar, or a rotating, radiation-emitting star.

The Sam Spade in the investigation, physicist and Nobel laureate Sam Ting of the Massachusetts Institute of Technology, said he expects a more definitive answer in a matter of months. He confidently promised: "There is no question we're going to solve this problem."

"It's a tantalizing hint," said California Institute of Technology physicist Sean Carroll, who was not part of the team. "It's a sign of something." But he can't quite say what that something is. It doesn't eliminate the other suspect, pulsars, he added.

The results from the Alpha Magnetic Spectrometer, or AMS, are significant because dark matter is thought to make up about a quarter of all the matter in the universe.

"We live in a sea of dark matter," said Michael Salamon, who runs the AMS program for the U.S. Energy Department. Unraveling the mystery of dark matter could help scientists better understand the composition of our universe and, more particularly, what holds galaxies together.

Ting announced the findings in Geneva at the European Organization for Nuclear Research, the particle physics laboratory known as CERN.

The 7-ton detector with a 3-foot magnet ring at its core was sent into space in 2011 in a shuttle mission commanded by astronaut Mark Kelly while his wife, then-Rep. Gabrielle Giffords, was recovering from a gunshot wound to the head. The device is transmitting its data to CERN, where it is being analyzed.

For 80 years scientists have theorized the existence of dark matter but have never actually observed it directly. They have looked for it in accelerators that smash particles together at high speed. No luck. They've looked deep underground with special detectors. Again no luck.

Then there's a third way: looking in space for the results of rare dark matter collisions. If particles of dark matter crash and annihilate each other, they should leave a footprint of positrons ? the anti-matter version of electrons ? at high energy levels. That's what Ting and AMS are looking for.

They found some. But they could also be signs of pulsars, Ting and others concede. What's key is the curve of the plot of those positrons. If the curve is one shape, it points to dark matter. If it's another, it points to pulsars. Ting said they should know the curve ? and the suspect ? soon.

The instrument will be measuring cosmic rays, where the footprints are found, until 2020 or so.

Other scientists praised the results and looked forward to more.

"This is an 80-year-old detective story and we are getting close to the end," said University of Chicago physicist Michael Turner, one of the giants in the field of dark matter. "This is a tantalizing clue and further results from AMS could finish the story."

(AP) ? NBC on Wednesday announced its long-rumored switch in late night, replacing Jay Leno at the "Tonight" show with Jimmy Fallon and moving the iconic franchise back to New York.

Fallon will take over in about a year, the switch coinciding with NBC's coverage of the 2014 Winter Olympics. Veteran "Saturday Night Live" producer Lorne Michaels also will take over as executive producer of "Tonight."

NBC made no announcement on who would replace Fallon at the 12:35 a.m. "Late Night" slot, although Seth Meyers of "Saturday Night Live" is considered a strong candidate.

The change at "Tonight," the longest-running and most popular late-night talk show, had been widely reported but not confirmed by the network until Wednesday. NBC reportedly just wrapped up negotiations with Fallon on a contract extension.

Steve Burke, chief executive officer of NBC Universal, said the network is purposefully making the move when Leno is still at the top of the ratings, just as when Leno replaced Johnny Carson at "Tonight" in 1992.

"Jimmy Fallon is a unique talent, and this is his time," Burke said.

Leno, in a statement, offered his congratulations to Fallon.

"I hope you're as lucky as me and hold on to the job until you're the old guy," he said. "If you need me, I'll be at the garage."

Fallon said, "I'm really excited to host a show that starts today instead of tomorrow."

Emotions were mixed among people waiting outside Leno's Burbank, Calif., studio to attend the taping of Wednesday's "Tonight" show.

"We love you, Jimmy!" said Natalie Renfro, 45, of Salt Lake City. But she gave a shout out to Leno, too: "I'll miss that big chin!"

Ryan Kelly, 39, of Los Angeles, said he's a Leno fan but added that the comedian has "had a good run. ... It's probably time for a fresh face. He's done a good job and I'm sure he'll pop up on TV somewhere else."

As for switching to Fallon, "I'll give it a shot," Kelly said.

Leno couldn't resist a jab at NBC in his monologue Wednesday, even as he as he lauded Fallon as "a hell of a guy" who is going to do a "great job."

"I just have one request for Jimmy: We've all fought, kicked and scratched to get this network up to fifth place. Now we have to keep it there. Jimmy, don't let it slip into sixth!" Leno joked, according to an NBC transcript.

Fallon took a puckish approach in his monologue.

"Welcome! This is 'Late Night with Jimmy Fallon ... for now," he said. "You guys probably heard the news: I'm going to be taking over the 'Tonight Show' next February! But don't worry. Until February, our focus is right here on whatever this show is called."

On his "Late Show" Wednesday on CBS, David Letterman feasted on NBC's announcement.

"Jay Leno now is being replaced, and this is the second time this has happened," he said in his monologue. "I mean, it's crazy. He's being replaced by a younger late night talk show host ? what could possibly go wrong? Honestly. They had pretty good luck with this in the past."

Later, Letterman offered a backhanded salute to Leno that wished him well with his stand-up appearances.

"But good luck to Jay. I know he'll be out on the road, getting it done and taking care of business. And congratulations on a nice long run there at the 'Tonight Show,' if, in fact, you're not coming back," Letterman said, according to a CBS transcript.

He devoted his Top 10 list to "things we'll miss about Jay Leno," including this at No. 4: "Can't remember the name of the bit, but it's the one where Jay is walking."

NBC has been quietly building a new studio for Fallon at its Rockefeller Center headquarters. "Tonight" began in New York in the 1950s, but Carson moved it to California in 1972. Starting next year, Fallon, Letterman, Jon Stewart and Stephen Colbert will tape late-night shows in New York. ABC's Jimmy Kimmel and TBS's Conan O'Brien will be the top California-based shows.

"The 'Tonight' show will bring even more jobs and economic activity to our city, and we couldn't be happier that one of New York's own is bringing the show back to where it started, and where it belongs," said New York Mayor Michael Bloomberg.

New York state recently added a tax credit in its budget that seemed designed specifically to benefit NBC's move east with "Tonight."

While a storied part of television tradition, the network late-night shows find themselves with much more competition now with cable programs like "Adult Swim," smaller talk shows hosted by Chelsea Handler and the Comedy Central duo of Stewart and Colbert, and a device ? a large number of people take that time to watch programs they had taped earlier on their DVRs.

NBC is worried that Kimmel will establish himself as a go-to late night performer for a younger generation if the network doesn't move swiftly to install Fallon. ABC moved Kimmel's time slot to directly compete with Leno earlier this year.

But the move also has the potential to backfire with Leno's fans, who did not embrace O'Brien when Leno was temporarily moved to prime time a few years ago.

"The guys at NBC are not totally stupid and are not going to shoot themselves in the foot," said Gary Carr, senior vice president and executive director of national broadcast for the ad buying firm TargetCast. "I think it's a good move for them long-term. But it may have short-term ramifications."

NBC has long prided itself on smooth transitions, but that reputation took a hit with the short-lived and ill-fated move of O'Brien to "Tonight" and Leno to prime time. In morning television, the "Today" show has taken a ratings nose dive in large measure because of anger at how Ann Curry was treated when she was ousted last year as Matt Lauer's co-host.

The Leno-Fallon changeover didn't begin smoothly. Leno had been cracking jokes about NBC's prime-time futility, angering NBC entertainment chief Robert Greenblatt, who sent a note to Leno telling him to cool it. That only made Leno go after NBC management much harder.

The first public effort toward making the transition smooth came Monday night, when Leno and Fallon appeared in a comic video making fun of the late-night rumors. It aired in between each man's show.

John Dawson, general manager for five NBC affiliates that have extensive reach throughout Kansas, said it will be difficult to give up a program that wins its time period by 33 percent.

"Jay has always been a great friend to the affiliates," he said. "For that alone it will be hard to give up."

But he said he believes in Fallon and in NBC's corporate owner, Comcast Corp., the nation's largest cable company.

"Comcast certainly knows how to launch entertainment programming," Dawson said.
